Integrity Home Care & Hospice Scholarship

Eligibility Requirements

• Applicant must be an enrolled Ozarks Tech student.
• Applicant must maintain a minimum Ozarks Tech cumulative GPA of 2.5.
• Applicant must be an employee of Integrity Home Care & Hospice.
• Applicant must be pursuing one of the following Allied Health programs:
  Occupational Therapy Assistant
  Physical Therapy Assistant
  Licensed Practical Nursing
  Associate of Nursing
• Priority will be given to applicants who have already been accepted or are currently enrolled in one of the programs listed above.

Renewal Criteria

• The scholarship is renewable between semesters, provided the recipient continues to meet all eligibility requirements.
• Renewal is not guaranteed and is subject to fund availability.

Use of Funds

• Scholarship funds may be applied toward tuition, fees, and/or required course materials at Ozarks Tech.
• Award amounts are dependent upon fund availability.

Refundable

• If a student receives other sources of financial aid, any surplus funds from this scholarship award will be directly refunded to the student.
